Aging Life Care Manager
Salary: $45 - $75/Hour DOE
Location : Jackson & Josephine Counties

Essential functions :
• Provision of health care management, advisory, and advocacy services to designated
members.
• Serves as the single point of contact for client's, families, advisors, and caregivers to
convey objective information and coordination of care among multiple providers.
• Strives to facilitate a relationship with client's, families, providers, and others,
promoting improved communications and fostering clients greater well-being,
independence, and peace of mind.
Responsibilities :
• New client admission
• Full comprehensive assessment and subsequent screening assessments; create plan based on assessment
• Home visits
• Assistance with monitoring medication logs and care plan compliance
• Monitoring client health status and well-being, recommending resources and
solutions as appropriate
• Transportation of clients for health care services and accompaniment to appointments when requested
• Liaison and coordinator with family members, designates, and providers; creation and maintenance of client’s personal health care record.
• Performance of administrative tasks associated with service to client's, including: scheduling, logging, and documenting all appointments and services; providing information, notices, and regular communications to clients and designates; and fulfillment of all designated tasks in a thorough, timely, and reliable manner
• Promotes company to referral sources, vendors, etc.
• Provides client education
• Participate in marketing efforts with goal of educating the public, cultivating business and securing new clients.

• Knowledge of the health care system, adept at facilitating health care coordination.
• Skilled in assessing client needs and in identifying resources and solutions.
• Strong interpersonal and communications skills, assertiveness, professionalism, and a
positive, upbeat, and patient disposition.
• Able to establish credible and trusted relationships with clients, family, other
designees, and providers alike.
• Able to relate easily and compassionately with seniors and families, provide non-directive
leadership to guide clients decision-making, facilitate constructive
relationships, and mediate and resolve conflict.
• Able to navigate clearly and discern accurately the boundaries of one’s role: as an
enabler of the actions and decisions of others, not the decision-maker, as a facilitator
of health care, not a provider. (Manage time efficiently)
• Discreet, ethical, careful in safeguarding confidentiality per HIPAA, attentive to detail,
prompt and timely, and committed to customer service. Services adhere to ALCA’s
Standard of Practice and Pledge of Ethics.
